Bij pati\u00ebnten met hartfalen en ijzertekort<\/a> zijn orale ijzersupplementen niet effectief vanwege een zeer slechte opname. Daarom is intraveneuze ijzersuppletie de enige behandeloptie waarvan is aangetoond dat deze veilig en effectief is voor pati\u00ebnten met hartfalen en ijzertekort bevestigd door bloedonderzoek. Het beste bewijs bestaat voor het intraveneuze ijzerpreparaat ferricarboxymaltose.<\/p>\n

De dosis intraveneuze ijzercarboxymaltose die aan een pati\u00ebnt wordt toegediend, wordt berekend op basis van lichaamsgewicht en hemoglobinegehalte en varieert tussen 500 en 2000 mg. Als de totale berekende vereiste dosis ijzer hoger is dan 1000 mg, wordt de toediening verdeeld in twee sessies, met een tussenperiode van 4-6 weken. IJzercarboxymaltose wordt toegediend als een 15 minuten durend intraveneus infuus van ijzer opgelost in 100 ml zoutoplossing. Deze behandeling kan veilig worden toegediend, zowel in het ziekenhuis als thuis.<\/p>\n

Het is bewezen dat intraveneuze ijzersuppletie bij pati\u00ebnten met hartfalen en ijzertekort de symptomen van hartfalen vermindert, de levenskwaliteit verbetert, de inspanningscapaciteit en skeletspierkracht verhoogt en het risico van herhaalde ziekenhuisopnames vermindert.<\/p>\n

Het effect van intraveneuze ijzersuppletie op de ijzerstatus moet worden gecontroleerd met een bloedtest, ongeveer 3 maanden na de laatst toegediende dosis. Nadat een pati\u00ebnt de volledige dosis heeft ontvangen, moet periodieke screening op ijzertekort worden uitgevoerd volgens de aanbeveling van de arts.<\/p>\n
