As you begin to prepare an advance care plan, it is important to have accurate, up-to-date information about your condition. The best person to provide you with this will be your doctor or nurse or other medical staff who are involved in your care.

So that you get to know what might be involved in your care, you may like to ask your doctor or nurse similar questions to those listed below:

  • Is my heart failure still well controlled with the medicines I am taking?
  • Is my heart failure getting worse?
  • What other options are available to me if my medicines stop having a satisfactory effect on my heart failure?
