Advance care directive

An advance care directive is a document in which you, or another person that you appoint, can provide guidance about your future medical care. It should provide clear directions of your wishes, including what treatments should be given to you in certain situations, and can include a living will.

Your doctor or nurse or a heart failure patient support group can guide you in preparing an advance care directive.
